Tell us your name and phone number, and the best time to call, and we will call you to confirm your registration.
Please note that we are not yet set up to accept online payments and this web site does not support secure transactions. Please do not email confidential information.
2. Print and mail or fax (613)549-7098 registration form found below.
A .pdf (Adobe) file will open or be available to save. Please print, complete and mail the form along with your cheque. Please do not send cash through the mail. Please note that you may need to obtain or update your free Adobe Reader - available here.
3. In person : Stop by CMHA Kingston at 6 Ordnance Street or call (613) 549-7027 durng business hours.
We will be more than happy to accept cash or a cheque from you at the branch.
